NOTE: To make a permanent record of a specific violation of behavior which has resulted in a disciplinary procedure: A copy of the written reprimand is forwarded to Human Resources for retention in the employee's permanent personnel file. The original memo should go to the employee and the supervisor or manager should retain a copy for his/her files.
