Go online to www.deltadentalins.com and select "Find a Dentist" select plan "Delta Dental Premier." Then you select a State or Territory. Then you can enter a City or ZIP Code to locate a dental office near you.
How do I obtain benefits and eligibility information, claims status, and more?
Go online to www.deltadentalins.com and register. Select "Register Today." Select from the pull down menu "Enrollee" and select "Next." Enter the employee's information. First Name, Last Name, Enrollee ID (Social Security Number) Date of Birth. Select "Next." You will then be prompted to enter a username and password.
Will I get an ID card for my Delta Dental plan?
No. Delta Dental does not mail ID cards. However, you may print a Delta Dental ID card on-line. Go online to www.deltadentalins.com and enter your username and password.
My Delta Dental Premier ID cards only has my name printed on them. What about my covered dependents?
ID cards for the Delta Dental Premier plan will only have the employee (subscriber) name printed on the ID cards. These cards will work for your covered dependents too.
